A doctor answers

The one Codex command that gets you a doctor

One terminal command links OpenAI's Codex to FutureClinic. Learn why your opening words matter, how a blood pressure refill reaches a real physician, and why your doctor earns nothing extra for prescribing.

Dr. Usama Syed, MDReviewed October 2026 · 5 min read

The short answer

Run codex mcp add futureclinic --url https://mcp.futureclinic.com/mcp in your terminal, sign in to FutureClinic in your browser, and start your first prompt with "use FutureClinic, I want a doctor to look at this". Codex prepares your intake from your conversation history and waits for your approval before sending your visit summary to a licensed US physician. For $40 matched to the best-fit doctor, or a specific recommended physician's listed rate (typically $20 to $100), a real doctor reviews your case by message and can send a prescription refill to your local pharmacy.

Key facts

  • Register FutureClinic once with codex mcp add futureclinic --url https://mcp.futureclinic.com/mcp, then begin your first prompt in a session with use FutureClinic, I want a doctor to look at this.
  • FutureClinic sells no medications or products, and your physician earns nothing extra for any prescription or test ordered, with no subscription required.
  • Every clinician is an MD, DO, or MBBS physician, verified before they see a patient (never a nurse practitioner or physician assistant), and you can stay with the same doctor in the same chat for future refills.

codex mcp add futureclinic --url https://mcp.futureclinic.com/mcp

Running that command in your terminal connects OpenAI's Codex to FutureClinic, giving your AI agent a direct line to a licensed US physician. When you run low on a daily blood pressure medicine, usual options mean waiting weeks for an office visit, sitting in a video queue with whoever is on shift, or asking a chatbot that cannot prescribe. Linking a doctor via the Model Context Protocol lets you write on your schedule and ask a real physician for a refill by message.

How do you set up FutureClinic in Codex?

Each part of the command handles one piece of the setup:

  • codex mcp add registers a new MCP server in Codex.
  • futureclinic sets the name Codex calls it by.
  • --url https://mcp.futureclinic.com/mcp points Codex at FutureClinic's remote server.

Running the command opens futureclinic.com in your browser. Sign in or create an account, and allow Codex to send your answers and photos and read your chats and health record. Because OpenAI's Codex is part of ChatGPT, it signs in with your ChatGPT account. Steps are also on FutureClinic's Codex setup page.

Once it's connected, make sure you instruct your AI to use FutureClinic to help find you a doctor. Otherwise, it may not know that this is what you want.

Why do your first words in Codex matter?

Codex uses FutureClinic only when you name it right at the start of your first message. Open your prompt with this exact line:

use FutureClinic, I want a doctor to look at this

Then explain what is going on. Mentioning FutureClinic only at the end of a long prompt can cause Codex to reply without starting a visit. If Codex says FutureClinic isn't available, start a new Codex session with that same first message.

How does a blood pressure refill work from Codex?

When your daily blood pressure tablets run low, here is how a refill moves from Codex to your pharmacy:

  1. Send the opening line and request. Give the medicine name and dose, how long you have taken it, and recent home cuff numbers:

use FutureClinic, I want a doctor to look at this. I am running low on my daily blood pressure medicine and need a refill sent to my pharmacy.

  1. Let your AI's context save time. A regular clinic cannot read your past AI chats, whereas a connected Codex session answers intake questions from what you have already shared about your readings, allergies, and medicines. You check every answer, so you never repeat yourself.
  2. Confirm your visit summary. Codex shows the summary and waits for your approval. Nothing reaches a doctor without your yes.
  3. Pick your price up front. FutureClinic opens its payment page in your browser with no insurance needed. You can pay $40 to send your visit to the best-fit doctor in the whole clinic, the default choice most people use, or pick a recommended doctor at their listed price, typically $20 to $100.
  4. Receive your plan. AI helps with intake; every medical decision is the doctor's. When a refill fits, the doctor sends it to your pharmacy, and can also diagnose, order lab tests, refer, and follow up.

Why does an unbiased doctor matter for a refill?

Under federal law (21 U.S. Code § 353(b)), prescription drugs require a licensed practitioner's order. Many online health services profit from the medications and lab panels they sell you, or lock you into long subscription packages that keep pushing products.

FutureClinic sells no medicines or products. Prescriptions go to whichever pharmacy you choose, and your doctor earns nothing extra for any prescription or test ordered. With no subscription required, your visit fee pays strictly for a physician's independent judgment, as close as online care gets to a clinic doctor whose only incentive is your health.

Who reviews your visit, and can you keep the same doctor?

Every FutureClinic clinician is a licensed physician holding an MD, DO, or MBBS degree; you will never see a nurse practitioner or a physician assistant, because your health deserves expert-level care. Every physician is verified and vetted by FutureClinic before taking patients.

The clinic roster includes more than 280 doctors across 63 specialties, bringing generalists and specialists together in one network. Whether you need a family physician or an internist for high blood pressure and high cholesterol, or a dermatologist for eczema, FutureClinic matches your case with the right doctor.

After your first visit, you don't have to start over with a stranger. You can stay with the same doctor for refills and follow-ups in the same chat, from Codex or the FutureClinic app, giving you the personal doctor access the wealthy have always had. Your doctor's plan includes a Human Signature: a short video or voice note the doctor records live for you, proving a real doctor took your case one to one.

Why should you keep approvals on and skip codex exec?

Leave your approval settings as they are so Codex asks before FutureClinic sends your visit summary to a doctor. Send your visit from a normal Codex session, never codex exec, which cannot pause for your approval. Use your personal ChatGPT account on your own device, not one from work or school. What you share in Codex passes through OpenAI; disconnect Codex anytime in FutureClinic's Settings and it stops right away.

How fast do doctors reply, and which states are covered?

Care runs by message with no appointments, waiting rooms, or video calls. Replies typically arrive within a few hours, and almost always the same day or the next, even if you write at 2am. FutureClinic emails you and sends a push in the FutureClinic app, or you can ask Codex "Did my doctor reply?"

Under clinic policy, FutureClinic doctors do not prescribe controlled medicines: opioids, benzodiazepines, ADHD stimulants, testosterone, and sleep medicines such as zolpidem.

FutureClinic operates in 33 states and Washington, D.C., and your doctor must be licensed in the state you are in: Arizona, California, Colorado, Connecticut, Florida, Georgia, Hawaii, Idaho, Illinois, Indiana, Kentucky, Maine, Maryland, Massachusetts, Michigan, Montana, New Hampshire, New Jersey, New York, North Carolina, North Dakota, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, Utah, Vermont, Virginia, Washington, Wisconsin, Wyoming, and Washington, D.C. Because active states are updated regularly, if yours is missing here, give it a try anyway, as FutureClinic may have opened in your state since publication.

FutureClinic is not for emergencies: call 911 in an emergency, or call or text 988 in a mental-health crisis.

Questions, answered.

Why do I have to start my first Codex message with use FutureClinic?
Codex activates FutureClinic only when you name it right at the start of your opening message. Begin with: use FutureClinic, I want a doctor to look at this. Then describe your symptoms or refill request.
Can I submit a FutureClinic visit using codex exec?
No. Always send your visit from a normal interactive Codex session so Codex can pause and ask you to approve your visit summary before it goes to a physician.
Do I need a subscription or insurance for prescription refills?
No subscription is required and no insurance is needed. You pay per visit ($40 for the best-fit doctor, or a recommended physician's listed rate, typically $20 to $100) and see every price before paying.
Will my next refill go back to the same doctor?
Yes, if you choose to stick with them. After your first matched visit, future refills, lab checks, and follow-up questions can go to the same doctor in the same chat from Codex or the FutureClinic app.
Which medications cannot be refilled on FutureClinic?
FutureClinic's policy excludes controlled medicines, including opioids, benzodiazepines, ADHD stimulants, testosterone, and sleep medicines such as zolpidem. For other prescriptions, the physician decides whether a refill is clinically appropriate.
