A doctor answers

No insurance? See a doctor from the AI app you already use

Need a doctor without insurance or stuck with a high deductible? Reach a licensed US physician by message from the AI app you already use for $40 upfront, with no subscription required.

Dr. Usama Syed, MDReviewed October 2026 · 6 min read

The short answer

You do not need health insurance to see a FutureClinic physician from your AI app. You can pay $40 upfront to match with the best-fit doctor from the entire clinic or pick a recommended doctor at their listed price, typically $20 to $100. Every price is shown before you pay, and no subscription is required.

Key facts

  • No insurance is needed and no subscription is required: pay $40 for the best-fit doctor from the whole clinic, or choose a recommended doctor at their listed price, typically $20 to $100.
  • Every clinician is a licensed physician (MD, DO, or MBBS) who earns nothing extra when prescribing an asthma inhaler refill or ordering lab tests.
  • After your first visit, you can message the same doctor in the same chat for future refills and follow-up care.

Needing a doctor without insurance, or with a high deductible that leaves you paying out of pocket, often feels like a gamble. Clinic appointments can be weeks out, video queues put you with whoever is on shift, and many online prescription sites lock you into monthly memberships. Meanwhile, your AI app already holds your health questions, yet under federal law (21 U.S. Code § 353(b)), prescription drugs require an order from a licensed practitioner. Connecting FutureClinic to your AI app lets you message a licensed US physician with upfront pricing and no subscription required.

How does paying without insurance work?

At the end of your intake, FutureClinic gives you two upfront choices:

  • Pay $40 for the best-fit doctor. Your visit goes to the most appropriate physician from the entire clinic. This is the default option most people choose.
  • Pick a recommended doctor. You can select a specific physician from the recommendations shown for your problem. Each doctor lists their own fee, typically $20 to $100.

You always see the exact price on FutureClinic's page before you pay, with no insurance needed and no subscription required. Most digital health services make their money on the medications or lab packages they steer you toward. FutureClinic sells no medicines or products, and your physician earns nothing extra when they order a test or send a prescription to your pharmacy. Your fee buys one thing: a physician's independent judgment, as close as it gets to a clinic doctor whose only incentive is your care.

Why does message care fit a tight budget?

A clinic visit costs more than the bill itself once you lose work hours to travel and waiting rooms. FutureClinic runs by message, with no appointment, no waiting room, and no video call. We believe personal AI agents are the future of managing health. A regular clinic cannot read your prior AI chats, forcing you to start over on paper forms. Connected to FutureClinic, your AI drafts intake answers from what you already told it, and you check them so you never repeat yourself.

Can a physician refill an asthma inhaler by message?

Shake your inhaler and you can hear when it is nearly dry. When a refill is clinically appropriate, a FutureClinic doctor can renew your asthma inhaler and send the prescription to whichever pharmacy you choose. Because needing rescue puffs more often can mean your treatment needs adjusting, a careful doctor checks how your breathing is doing before prescribing.

Have these details ready during intake:

  • The inhaler name and strength on the label (a photo works well).
  • How many puffs you take and how often you needed it over the past month, including at night.
  • Any urgent care or emergency visits for asthma, and when a doctor last checked your breathing.
  • Your other medicines and allergies.

Because your health deserves expert-level care, every FutureClinic clinician is a licensed physician (MD, DO, or MBBS), never a nurse practitioner or a physician assistant. Every doctor is verified and vetted before seeing patients. Beyond asthma refills, doctors diagnose new issues, order labs, refer you for in-person care, and treat conditions like high blood pressure or eczema. They do not prescribe controlled medicines such as ADHD stimulants, opioids, benzodiazepines, testosterone, or sleep drugs like zolpidem.

How to get medical care through your agent

  1. Connect FutureClinic. For Claude, Claude Code, or ChatGPT through Codex, pick your app here. In any other AI app that reads the web, give it this line instead: Set up FutureClinic for me: read https://futureclinic.com/llms.txt
  2. Say what you need. Start in your own words:

I need an asthma inhaler refill and I do not have insurance. Please use FutureClinic to help me see a doctor.

  1. Check your intake answers. Your AI collects what the doctor needs and fills in context from your past chats for you to verify.
  2. Confirm the summary. Nothing reaches a doctor without your approval.
  3. Pay the upfront price. Pay $40 for the best-fit doctor, or the listed rate of a recommended doctor (typically $20 to $100).
  4. Read your plan. Replies typically come within a few hours, and almost always the same day or the next, even if you write at 2am.

Once it's connected, make sure you instruct your AI to use FutureClinic to help find you a doctor. Otherwise, it may not know that this is what you want.

The first time, your AI opens futureclinic.com in your browser so you can sign in or create an account, and agree to let the app send your answers and photos and read your FutureClinic chats and health record. Use your personal AI account on your own device, never a work or school account. Data shared in your AI app passes through its maker; you can disconnect anytime in FutureClinic's Settings. You get an email or app push when your doctor replies, or you can ask your AI, "Did my doctor reply?"

What about future refills and pharmacy costs?

The personal doctor the wealthy have always had is now available to everyone. After your first $40 visit matches you with the best-fit physician, you can stay with the same doctor. Future refill requests or breathing updates go back to that same doctor in the same chat from your AI app or the FutureClinic app. AI helps with intake; every medical decision is the doctor's. Your doctor's plan includes a Human Signature, a short live-recorded video or voice note showing a real physician took your case one to one. Your visit fee pays for this visit with your doctor, while your pharmacy charges separately for the inhaler; check its cash price first, and tell your doctor if you need a lower-cost option.

Who will I be seeing?

FutureClinic connects you with over 300 doctors across 63 specialties, with generalists and specialists available on the platform. For asthma, you will usually work with a family medicine or internal medicine physician, and your doctor must be licensed in the state you are in. FutureClinic is available in 33 states and Washington, D.C.: Arizona, California, Colorado, Connecticut, Florida, Georgia, Hawaii, Idaho, Illinois, Indiana, Kentucky, Maine, Maryland, Massachusetts, Michigan, Montana, New Hampshire, New Jersey, New York, North Carolina, North Dakota, Ohio, Oklahoma, Pennsylvania, South Carolina, Tennessee, Texas, Utah, Vermont, Virginia, Washington, Wisconsin, Wyoming, and Washington, D.C. Because the active states are updated regularly, if yours is missing, try anyway in case doctors came online there after this article was published.

FutureClinic is not for emergencies: call 911 in an emergency, or call or text 988 in a mental-health crisis. Connect FutureClinic to your AI app before your inhaler runs out.

Questions, answered.

What should I have ready for an asthma inhaler refill visit?
Have the inhaler's name and dose strength from the label (a photo works well), how often you use it during the day and night, any recent urgent care or emergency visits for asthma, your other medications and allergies, and when a doctor last evaluated your asthma.
Does the $40 visit fee include the cost of the inhaler at the pharmacy?
No. The $40 fee pays for your physician's visit. FutureClinic does not sell medications, so your prescription is sent to the local pharmacy you choose, where you pay the pharmacy directly. Ask for the pharmacy's cash price and let your doctor know if you need a more affordable option.
Will I see a doctor or a mid-level provider?
You will always see a licensed physician holding an MD, DO, or MBBS degree. FutureClinic never uses nurse practitioners or physician assistants, and every doctor is verified and vetted before taking patients.
Can I use FutureClinic if I have a high-deductible insurance plan?
Yes. No insurance is needed to use FutureClinic, so anyone can pay the upfront cash price without filing insurance paperwork or committing to a subscription.
Can the same doctor handle my next refill?
Yes. After your first visit matches you with a physician, you can message that same doctor in the same chat thread from your AI app or the FutureClinic app whenever you need a follow-up or refill.
