ED treatment online: generic pills, a doctor by message, no subscription

Do you want an ED subscription and everything tied to it, or an expert doctor directly, one to one? For a $40 visit on FutureClinic, the same physician can prescribe generic sildenafil or tadalafil to your pharmacy.

The short answer

When you look for ED treatment online, the first question to ask is whether you want a monthly pill subscription and everything tied to it, or direct, one-to-one care from an expert physician. If you want a brand's compounded chewables or pills priced into a monthly plan, services like Ro, Hims, or BlueChew are strong options. If what you actually wanted was a licensed doctor (MD, DO or MBBS) who can prescribe generic sildenafil or tadalafil to the pharmacy you choose, with several months of refills when safe and no subscription required, a FutureClinic visit is $40 for the best-fit doctor (or typically $20 to $100 to pick a specific doctor).

Key facts

  • Ask whether you want a monthly pill plan or a doctor one to one: if you want compounded combination tablets or a bundled subscription, Ro, Hims, and BlueChew are strong options, while FutureClinic connects you directly with a physician (MD, DO or MBBS) for $40 (or a chosen doctor's price, typically $20 to $100) with no subscription required.
  • On FutureClinic, your doctor sells no medicine and earns nothing extra for any prescription or test, and for a stable medication they can prescribe several months at once to the pharmacy you choose: as of October 2026, a $40 visit plus 90 tablets of generic sildenafil 100 mg or tadalafil 20 mg ($10.07 plus $5.25 shipping at Mark Cuban Cost Plus Drugs) compares with $176 a month for 4 doses of as-needed tadalafil on Ro.
  • You receive a Human Signature (a live video or voice note from your physician) and can message the same doctor over time as you adjust doses or check the morning testosterone level the American Urological Association recommends, with every FutureClinic doctor you see working from one medical record.

When searching for ED treatment online, ask one question first: do you want a monthly pill subscription and all its requirements, or direct, one-to-one care from an expert doctor? If you want what subscription brands sell beyond the doctor, such as compounded chewables or pills bundled into a monthly plan, they are a strong option. If all you wanted was an expert doctor directly, FutureClinic gives you that with nothing attached and no subscription required.

On FutureClinic, a specific physician with an MD, DO or MBBS degree reads your case and answers you, never a nurse practitioner or physician assistant. No form is approved in bulk. Your doctor's plan includes a Human Signature, a short video or voice note recorded live for you. Real care takes hours, not seconds: replies typically come within a few hours, and almost always the same day or the next.

You can message the same doctor over time in the same chat, however long in between, to build trust, adjust doses, and track progress. Your doctor can also help with other problems in their specialty, like blood pressure or cholesterol, and every FutureClinic doctor you see across generalists and specialists works from one medical record.

What changes when you work with the same doctor on ED?

Erectile dysfunction is trouble getting or keeping an erection firm enough for sex. NIDDK reports 30 million to 50 million US men have ED (40% at age 40; 70% by age 70). Causes include diabetes, vascular disease, high blood pressure, kidney disease, obesity, low testosterone, prostate enlargement, medicines, and anxiety.

The American Urological Association (AUA) guideline emphasizes shared decision-making and titrating doses up or down (large meals or lack of sexual stimulation cause many failures). With the same doctor, you can step sildenafil between 25 mg, 50 mg, and 100 mg, or switch to daily tadalafil (2.5 mg or 5 mg; daily 5 mg is also the dose for benign prostatic hyperplasia). The AUA recommends measuring morning total testosterone (deficiency is below 300 ng/dL with symptoms) and says glucose or A1c and lipids may possibly change ED care, which your doctor can order at discounted prices; because ED is a risk marker for heart disease, the AUA calls the diagnosis a chance to discuss heart risk.

FutureClinic sells no medicine, and your doctor earns nothing extra for any prescription or test, so they never upsell pills or lab panels. If your doctor decides it's right for you, they can prescribe any non-controlled brand or generic to the pharmacy you choose, with no house formulary.

FutureClinic has over 300 doctors caring for patients by message. Those physicians span more than 60 specialties.

AI ED treatment: which FDA-approved pills a doctor can prescribe by message

Four FDA-approved oral PDE5 inhibitors treat ED and work about equally well with sexual stimulation:

  • Sildenafil: Per the Viagra label, usual dosing is 50 mg about 1 hour before sex (25 to 100 mg, max once daily, 4-hour half-life; 25 mg start at age 65+ or with liver, kidney, or CYP3A4 issues). Sildenafil 20 mg is generic Revatio.
  • Tadalafil: Per the Cialis label, as-needed dosing starts at 10 mg (5 to 20 mg, max once daily) and lasts up to 36 hours (17.5-hour half-life); daily dosing is 2.5 mg or 5 mg. The label says not to split Cialis tablets.
  • Vardenafil and Avanafil: Vardenafil starts at 10 mg about 60 minutes before sex (5 to 20 mg). Avanafil (Stendra) starts at 100 mg as early as 15 minutes before sex (no generic at Cost Plus Drugs as of October 2026).

Your doctor reviews your history and home blood pressure. All four are contraindicated with nitrates and guanylate cyclase stimulators (riociguat, vericiguat), need caution with alpha-blockers, CYP3A4 inhibitors, liver or kidney disease, or age 65+, and should not be used when a heart condition makes sex inadvisable. Common side effects include headache, flushing, indigestion, nasal congestion, back or muscle ache, visual changes, and dizziness. Testosterone is a Schedule III controlled substance (21 CFR 1308.13(f)); FutureClinic never prescribes controlled medicines, and testosterone alone does not effectively treat ED.

Ro, Hims, and BlueChew fit better if you want their compounded products (Ro Sparks, Daily Rise Gummies, Hims' compounded tablets, or BlueChew's chewables and GOLD tablet with sildenafil, tadalafil, apomorphine, and oxytocin) or a monthly pill bundle. FutureClinic connects you directly with a physician who can prescribe FDA-approved pills, if your doctor decides it's right for you, to the pharmacy you choose.

What does ED treatment cost over a full year?

Low per-dose prices assume you stay subscribed every month to keep getting refills. On Ro as of October 2026, generic sildenafil 25 mg starts at $4/dose with an 8-dose monthly minimum ($384/year), 100 mg is $10/dose ($960/year at 8 doses/month), and as-needed tadalafil (10 or 20 mg) starts at $176/month for 4 doses ($2,112/year). BlueChew starts at $20/month ($240/year).

On FutureClinic, a visit is $40 for the best-fit doctor, or a chosen doctor's listed price, typically $20 to $100. For a stable medicine, your doctor can write several months at once (such as a 6-month script) when safe, sent to the pharmacy you choose, including mail-order pharmacies that deliver to your door, with a cash discount coupon.

As of October 2026, 90 tablets of sildenafil 100 mg or tadalafil 20 mg at Mark Cuban Cost Plus Drugs cost $10.07 (plus $5.25 shipping), and 90 tablets of tadalafil 5 mg cost $7.90. On GoodRx as of October 2026, 30 tablets of sildenafil 100 mg were $19.80 (retail $1,378) and tadalafil 20 mg were $14.24. A $40 visit plus two 90-tablet Cost Plus Drugs fills is about $71 for the year. You can also use insurance at the pharmacy (FutureClinic helps with prior authorizations), or join a doctor's optional membership, typically $25 to $100 a month for unlimited care with that doctor, cancel any time.

Questions, answered.

Can I get generic sildenafil or tadalafil delivered to my door with FutureClinic?
If your doctor prescribes it, the prescription goes to the pharmacy you choose, including local pharmacies for pickup and mail-order pharmacies that deliver to your door.
How much does an online ED visit cost on FutureClinic?
A visit is $40 when you let FutureClinic match you with the best-fit doctor, or you can select a specific physician at their listed price, which is typically $20 to $100. No subscription is required, and when a medication is stable and safe, your doctor can prescribe several months at once.
What is the difference between sildenafil and tadalafil?
Both are FDA-approved oral PDE5 inhibitors that work about equally well with sexual stimulation. Sildenafil (generic Viagra) is taken about 30 minutes to 4 hours before sex and lasts up to 4 hours, while tadalafil (generic Cialis) can work for up to 36 hours when taken as needed, or can be taken as a low daily dose of 2.5 mg or 5 mg.
Does FutureClinic prescribe testosterone for erectile dysfunction?
No. Testosterone is a Schedule III controlled substance under federal law, and FutureClinic physicians never prescribe controlled medications. However, your FutureClinic doctor can order a morning total testosterone blood test to check whether a hormone deficiency is contributing to your symptoms.
