A doctor answers

Zepbound or Wegovy online without a membership: what you actually pay

Before joining a GLP-1 site, ask: do you want a monthly membership program, or a doctor one to one? LillyDirect and NovoCare fill prescriptions from any doctor at the maker's price, with no subscription needed.

The short answer

Before signing up for a weight-loss website, it helps to ask: do you actually want a monthly membership program and everything that comes with it, or did you just want an expert doctor, directly, one to one? As of October 2026, Eli Lilly's LillyDirect (Zepbound from $299 a month; $449 for higher doses refilled within 45 days; Foundayo pills from $149) and Novo Nordisk's NovoCare (Wegovy pens $199 for the first two fills, then $349; Wegovy pills from $149) fill cash prescriptions from any state-licensed physician with no membership fee. If you want a company's bundled coaching or compounded product, those services are a strong option, while if you want a physician (MD, DO or MBBS) with nothing attached, FutureClinic lets you message the same doctor for $40 (or a chosen doctor's price, typically $20 to $100) with no subscription required.

Key facts

  • Ask whether you want a monthly weight program or a doctor one to one: if you want a company's bundled coaching or compounded formula, a membership site is a strong option, while if you wanted an expert physician (MD, DO or MBBS) with nothing attached, FutureClinic charges $40 for the best-fit doctor (or a chosen doctor's price, typically $20 to $100) with no subscription required.
  • LillyDirect and NovoCare accept prescriptions from any state-licensed doctor with no membership fee: as of October 2026, Zepbound vials and KwikPens cost $299 to $449 a month (when refilled within 45 days), Wegovy pens cost $199 for the first two fills then $349 ($399 for 7.2 mg HD), and Wegovy or Foundayo daily pills cost $149 to $299 a month.
  • Telehealth memberships such as Ro Body and Hims & Hers add $39 for the first month and up to $149 a month after ($1,678 in first-year membership fees as of October 2026) on top of brand-name drug costs, whereas on FutureClinic you work with the same doctor over time, receive a live-recorded Human Signature, and can get several months of a stable prescription at once when your doctor judges it safe.

When searching for Zepbound or Wegovy online, ask one question before joining a monthly weight program: do you actually want that service and its requirements, or did you just want an expert doctor, directly, one to one?

Eli Lilly (LillyDirect) and Novo Nordisk (NovoCare) fill prescriptions from any licensed physician at the maker's cash price with no membership fee (as of October 2026). If you want what another company sells beyond the doctor, like bundled coaching or its own compounded formula, it is a strong option. If you wanted an expert doctor, FutureClinic gives you one-to-one care with nothing attached and no subscription required.

On FutureClinic, every clinician is a physician (MD, DO or MBBS), never a nurse practitioner or physician assistant. A specific doctor reads your whole case, makes every medical call, and sends a Human Signature with your plan, a short video or voice note recorded live for you. No form is approved in bulk. Real care takes hours, not seconds: replies typically arrive within a few hours, and almost always the same day or the next.

Your chat stays open with that same doctor however long in between, building trust as you try treatments together and track progress: adjusting a dose while nausea settles, comparing a weekly pen with a daily pill, or reading new labs against earlier ones. FutureClinic sells no medicine, and your doctor earns nothing extra for any prescription or test, prescribing any non-controlled brand or generic to the pharmacy you choose with no house formulary. They also treat other problems in their specialty, like blood pressure or prediabetes, just as a dermatologist treating hair loss can treat acne or eczema. FutureClinic has over 300 doctors. You can see generalists and specialists across more than 60 specialties, and every FutureClinic doctor you see works from one medical record.

What do Zepbound, Wegovy, and Foundayo cost without a membership?

Through LillyDirect, a 28-day Zepbound (tirzepatide) supply of four vials (self-pay only) or one four-dose KwikPen includes free delivery or Walmart pickup (needles or syringes extra; as of October 2026): 2.5 mg (starting dose) is $299 a month; 5 mg is $399; and 7.5 mg to 15 mg is $449 under Lilly's Self Pay Journey Program when refilled within 45 days ($499 for 7.5 mg and $699 for 10 to 15 mg if late). Lilly's KwikPen Self-Pay Savings Card matches $299 to $449 at retail chains through December 31, 2026, while GoodRx (as of October 2026) shows the 2.5 mg KwikPen at $600.68 average retail and $299 with the maker offer. Lilly's daily pill Foundayo (orforglipron), FDA-approved April 1, 2026 and taken without fasting, costs $149 a month (0.8 mg, 2.5 mg), $199 (5.5 mg; 2.5 and 5.5 mg reduced through December 31, 2026), and $299 (9 mg, or 14.5 mg and 17.2 mg refilled within 45 days; $349 if late).

Through NovoCare delivery or local pickup with Novo's savings offer, Wegovy (semaglutide) cash prices against its $1,349.02 list price and $1,643.18 GoodRx average retail price (as of October 2026) are $199 per 28-day box of four pens for a new patient's first two fills of 0.25 mg or 0.5 mg through December 31, 2026, then $349 a month (0.25 mg to 2.4 mg), or $399 for Wegovy HD 7.2 mg. The 30-tablet morning empty-stomach pill costs $149 (1.5 mg), $199 (4 mg), and $299 (9 mg, 25 mg).

TrumpRx.gov lists the same maker prices (as of October 2026). Through the Medicare GLP-1 Bridge (July 1, 2026 to December 31, 2027), eligible Part D enrollees pay $50 a month for Wegovy, Foundayo, or the Zepbound KwikPen after prior authorization. With commercial coverage, maker cards lower copays to as little as $25 a month, and FutureClinic helps get prior authorizations.

What does a telehealth membership add over a year?

Lilly's telehealth directory lists independent weight options from $25 a visit to $149 a month (as of October 2026), excluding medicine.

Low first-month rates on weight sites assume you stay subscribed to keep getting prescriptions from them. On Ro's monthly plan or Hims & Hers (as of October 2026), membership fees total $1,678 in year one ($39 plus 11 months at $149) before medicine; Found charges $99 to $149 a month for brand plans ($169 to $289 with compounded medicine), and Mochi Health charges $79 a month plus the drug. Those services suit you if you want a company's own compounded formula (the FDA resolved the tirzepatide and semaglutide shortages in 2024 and 2025 and restricts routine copies) or medicines bundled into a monthly plan.

If you wanted an expert doctor directly, FutureClinic charges $40 for the best-fit doctor, or a chosen doctor's listed price, typically $20 to $100, with no subscription needed. During early dose steps, when the Endocrine Society suggests monthly checks for three months, you can pay per visit or join an optional membership (typically $25 to $100 a month) for unlimited care with that doctor, cancel any time. For a stable medicine, your doctor can prescribe several months at once (such as a 90-day NovoCare fill) when they judge it safe, to the pharmacy you choose, where insurance may cover it or a discount coupon may lower the cash price. Six $40 visits cost $240 in year one, versus $1,678 in membership fees.

How does a doctor prescribe and follow up by message?

Non-controlled weight medicines include Zepbound, Wegovy, Foundayo, Saxenda, Contrave, and Xenical (in main non-diabetes trials, average weight loss was 15.0% to 20.9% on Zepbound 5 to 15 mg at 72 weeks, 14.9% on Wegovy 2.4 mg at 68 weeks, 13.6% on the Wegovy 25 mg pill at 64 weeks, and 11.1% on Foundayo 17.2 mg at 72 weeks). FutureClinic doctors do not prescribe controlled medicines like phentermine or Qsymia.

In a July 2026 JAMA study (Healio), 75.6% of online GLP-1 sites shipped medicine without confirmation. At FutureClinic, we believe AI is a tool for doctors and the doctor is in charge of every case, because only an expert can tell when an AI makes a mistake or heads to the wrong answer. Our AI works behind the scenes like a chief resident, gathering your history into a summary you confirm first, and never gives medical advice by itself.

Your doctor checks label criteria (NIDDK notes BMI 30 or more, or 27 or more with a weight-related condition, plus diet and exercise), screens for contraindications (medullary thyroid carcinoma or MEN 2 history) and warnings (pregnancy, pancreatitis, gallbladder disease, or birth-control pill interactions with Zepbound and Foundayo), and orders discounted labs when needed. They send a prescription to the pharmacy you choose, including mail-order pharmacies that deliver to your door, LillyDirect, or NovoCare, if your doctor decides it's right for you.

Questions, answered.

Can any licensed doctor send a prescription to LillyDirect or NovoCare?
Yes. Both LillyDirect Pharmacy and NovoCare Pharmacy accept valid prescriptions from any state-licensed physician for FDA-approved uses. Once the pharmacy receives the prescription with your mobile number, it texts you a link to complete your self-pay order.
Why does Zepbound 7.5 mg to 15 mg cost $449 instead of $499 or $699?
Under Eli Lilly's Zepbound Self Pay Journey Program (as of October 2026), the 7.5 mg, 10 mg, 12.5 mg, and 15 mg vials and KwikPens cost $449 for a 28-day supply on the first fill and stay at $449 as long as you complete each refill purchase within 45 days of receiving your previous fill. If a refill is more than 45 days late, the price reverts to $499 for 7.5 mg and $699 for 10 mg to 15 mg.
What is the difference between the Wegovy pill and Foundayo?
Both are daily oral GLP-1 tablets approved for chronic weight management that start at $149 a month for cash-paying patients as of October 2026. The Wegovy pill (oral semaglutide, approved in December 2025) must be taken in the morning on an empty stomach with up to 4 ounces of plain water at least 30 minutes before food or other medicines, whereas Foundayo (orforglipron, approved in April 2026) can be taken any time of day with or without food.
How much does it cost to get refills on FutureClinic?
There is no subscription required on FutureClinic. You pay $40 for the best-fit doctor or a chosen doctor's listed price (typically $20 to $100) per visit, or, with most doctors, join an optional monthly membership (typically $25 to $100 a month) for unlimited care with that doctor, cancel any time. For a stable maintenance medicine, your doctor can also prescribe several months at once when they judge it safe.
