Asthma care online: inhaler refills and a plan from a doctor by message

Compare online and in-person asthma care, May 2026 GINA inhaler guidelines, and cash prices from $12.31 for albuterol. Message a physician ($40 a visit) for a written plan and refills if right for you.

The short answer

On FutureClinic, a specific MD, DO or MBBS reads your asthma case by message, replies with a live recorded Human Signature, and can update your written asthma action plan or send inhaler refills to the pharmacy you choose if your doctor decides it's right for you. A visit is $40 for the best-fit doctor, or typically $20 to $100 when you pick a specific doctor, with no subscription required. You can message the same doctor over time to track symptom control and adjust your inhalers together.

Key facts

  • A FutureClinic visit is $40 for the best-fit doctor (or typically $20 to $100 for a chosen physician) with replies typically within a few hours, compared with an average 31-day wait for a new-patient physician appointment across 15 US metro areas (AMN Healthcare, May 2025).
  • The May 2026 Global Initiative for Asthma (GINA) update advises against treating asthma in adults, adolescents and children 6 to 11 with an albuterol rescue inhaler alone, and messaging the same doctor over months lets you track symptom control and step inhaled corticosteroid doses up or down together.
  • FutureClinic doctors earn nothing extra for any prescription or test and send prescriptions to the pharmacy you choose; as of October 2026, generic albuterol HFA starts at $12.31 on Cost Plus Drugs and $19.45 on GoodRx, and AstraZeneca and Boehringer Ingelheim cap eligible brand-name inhalers at $35 a month.

Asthma care by message lets a physician review your symptom control, update your written action plan, and send inhaler refills to your pharmacy. In AMN Healthcare's May 2025 survey of 15 US metros, new-patient physician waits averaged 31 days (23.5 days in family medicine), and GINA notes that poor adherence to daily inhaled steroids leaves some patients on albuterol alone.

Many people seek online care for convenience without getting trapped in subscriptions or tied to one medication provider. Do you want another service and the requirements attached to it, or do you want an expert doctor of your own? If you want medicines bundled into a subscription, another service is a strong option. If you wanted an expert doctor directly, FutureClinic gives you that with nothing attached.

On FutureClinic, a specific doctor reads your whole case and answers you one to one, and your plan includes a Human Signature, a short video or voice note recorded live for you. No form is approved in bulk. If your doctor decides it's right for you, they can send inhaler refills to the pharmacy you choose and write an asthma action plan by message. That same doctor can treat other problems in their specialty, and every FutureClinic doctor you see works from one medical record.

FutureClinic is not for emergencies, and an asthma attack is an emergency: call 911, or call or text 988 in a mental-health crisis.

Why messaging the same doctor over time matters for asthma

Asthma shifts with seasons and triggers, so messaging the same doctor time after time, however long in between, lets you build trust, try treatments together, and track progress in one chat. Together you can compare home peak expiratory flow (PEF) readings against your personal best, review rescue puffs over the past four weeks, swap an inhaler that causes thrush or hoarseness, or review flare-ups and steroid courses over the past year, stepping down after two to three stable months or stepping up after a flare.

Your doctor's only incentive is to help you. FutureClinic sells no medicines, and your doctor earns nothing extra for any prescription or test, so they never upsell a medicine or lab panel. With no subscription required, they can prescribe whichever non-controlled medicine fits you, any brand or generic, to the pharmacy you choose, with no house formulary, including several months of a stable medicine when they judge it safe.

AI asthma treatment: what a doctor manages by message, and when to go in person

FutureClinic has over 300 doctors on the platform. Generalists and specialists span more than 60 specialties, and every clinician is a state-licensed, background-checked physician with an MD, DO or MBBS degree, never a nurse practitioner or physician assistant. First, an AI assistant trained by the doctors asks about your diagnosis, past spirometry or peak flow numbers, current inhalers, rescue canisters used in the past year, and recent steroid courses or urgent visits. The assistant never gives medical advice. You confirm a summary before the doctor reads the whole conversation and writes back. Real care takes hours, so replies typically come within a few hours, almost always the same day or the next.

Your physician can also treat related conditions in their specialty, such as allergic rhinitis or reflux. In-person care is required for physical exams, clinic spirometry, FeNO breath tests, office allergy shots, and the first dose of dust-mite sublingual immunotherapy (Odactra), per GINA and NAEPP 2020. If asthma stays uncontrolled at Step 4 despite good technique and adherence, GINA advises referral to a severe-asthma specialist, who may consider a biologic.

Why current guidelines move away from albuterol alone

The May 2026 GINA report estimates asthma affects 300 million people worldwide. Because people with infrequent symptoms can still have severe exacerbations, GINA avoids the term "mild asthma" and recommends against treating adults, adolescents, or children 6 to 11 with a short-acting beta-agonist (SABA, like albuterol) alone. Using three or more 200-dose SABA canisters a year is linked to more exacerbations and higher mortality. GINA outlines two tracks (while NAEPP 2020 recommends daily and reliever ICS-formoterol for ages 4 and up with moderate to severe persistent asthma):

  • Track 1 (ICS-formoterol reliever): Low-dose budesonide-formoterol is taken as needed at Steps 1 to 2 (55% fewer severe exacerbations and 65% fewer emergency visits or hospitalizations on average versus SABA alone in adult and adolescent trials), or daily plus as needed at Steps 3 to 5 (MART, 32% fewer severe exacerbations on average versus the same ICS-LABA dose). In the US, Symbicort and generic Breyna (approved March 2022) state they are "NOT indicated for the relief of acute bronchospasm," making reliever and MART use guideline-endorsed off-label care (max 12 puffs of 160/4.5 mcg in 24 hours for ages 12 and up; never pair with salmeterol or vilanterol).
  • Track 2 (ICS-SABA or SABA with an ICS): Uses as-needed albuterol-budesonide (Airsupra, FDA-approved for adults 18 and up at 2 puffs of 90/80 mcg, max 12 puffs in 24 hours, cutting yearly severe exacerbations by 53% versus albuterol alone in the BATURA trial) or a daily ICS or ICS-LABA with albuterol HFA. Shake suspension inhalers right before each puff. Tiotropium is a Step 5 add-on; montelukast carries a March 2020 FDA Boxed Warning for serious mental health side effects.

What does a doctor check before refilling an inhaler?

GINA checks four questions over the past four weeks: daytime symptoms more than twice weekly, any night waking, SABA use more than twice weekly (excluding pre-exercise puffs), and any activity limitation. Your doctor also checks flare-ups, smoking or vaping, pregnancy, and CYP3A4 interactions (ritonavir, ketoconazole, itraconazole, erythromycin, clarithromycin), reviews home PEF readings (best of three blows on one meter), and can order discounted labs like blood eosinophils.

How do message visits and inhaler prices compare with in-person care?

In 2003 to 2010 American Time Use Survey data, an office visit averaged 121 minutes (37 travel, 84 at the clinic) and $43 in lost time. In 2024 Medicare data, average new-patient submitted charges were $267.25 (code 99203, allowed $105.33) and $410.82 (code 99204, allowed $160.28). A FutureClinic visit is $40 for the best-fit doctor, or typically $20 to $100 for a chosen doctor, with no subscription required. Optional memberships (typically $25 to $100 a month) offer unlimited care with that doctor, cancel any time.

Questions, answered.

Can a doctor refill my daily controller and rescue inhalers by message?
Yes, if your doctor decides it's right for you after reviewing your asthma history, symptom control over the past four weeks, recent flare-ups, and past lung function records. For a stable asthma regimen, your physician can prescribe several months of medication at once when they judge it safe, sent to the pharmacy you choose, with no subscription required.
Why does GINA warn against using an albuterol inhaler by itself?
According to the May 2026 Global Initiative for Asthma (GINA) strategy report, treating asthma with a short-acting beta-agonist (SABA) like albuterol alone leaves underlying airway inflammation untreated and is associated with a higher risk of severe exacerbations and asthma-related death, even when daytime symptoms seem infrequent. GINA instead recommends treatment that includes an inhaled corticosteroid.
Can I use a $35 monthly inhaler cap or a discount coupon with a FutureClinic prescription?
Yes. Because FutureClinic doctors earn nothing extra for any prescription and send prescriptions to the pharmacy you choose rather than locking you into an in-house pharmacy bundle, you can use your regular pharmacy insurance, cash discount coupons, mail-order pharmacies, or manufacturer $35 monthly caps from AstraZeneca and Boehringer Ingelheim if you meet their eligibility rules.
What parts of asthma care require an in-person visit instead of messaging?
Clinic spirometry, exhaled nitric oxide (FeNO) breath testing, subcutaneous allergy shots, and the supervised first dose of sublingual dust-mite immunotherapy require an in-person office visit.
